Cancellation Form

In order to maximize our teachers' time, lessons that will be missed due to vacation or other schedule conflicts must be noted before the 25th of the previous month. For instance, if you will be out of town for two weeks in June, you need to let us know before the 25th of May in order to avoid being billed for those two lessons.

Please note that this form is not for emergency cancellations. If you need to cancel a scheduled lesson, please call or email your teacher to discuss reschedule options in accordance with our studio policy.
